PRESCRIPTION-BeneRx administered by NPA (National Prescription Administrators)

Customer Service: 1-800-467-2006

After the completion of your prescription enrollment application, your prescription coverage through BeneRx will become effective 60 days from your date of hire on the first day of the following month. The only exception, 10 month employees who begin work on the first day of school will receive prescription coverage effective their first day of employment.

If you do not enroll all eligible members of your family within 60 days of the time you or they first become eligible for coverage, you must wait until the next Open Enrollment period to do so. Open Enrollment for prescription is the same as medical, in the month of October.

An identification card will be issued to you. Please do not leave your card at the pharmacy. The card contains all the information your pharmacist needs, there is nothing else that you need to do and no claim forms to submit. You will be asked to pay a co-pay of $7.00 for name brands and $5.00 for generic brands.

PRESCRIPTION PLAN REMINDER:

BE AWARE OF DEPENDENT CHILD

PRESCRPTION STIPULATIONS

Do you have a dependent that turned 19 during 2005?

Please remember that under our prescription insurance, these dependents are only covered until the end of the year in which they turned 19. This means that their prescription insurance expired on December 31,2005. The only exception to this rule is if the dependentis a full time student at an accredited college, university or other form of higher education.

In January, your druggist will probably reject your request for a prescription for that child under the plan, unless you have sent in proof from the college/university that your child continues in the school.

If they dependent is attending college, etc., the coverage will not be renewed unless you show proof that they are full time students. A letter from the school indicating full time sstudent status, usually obtained from the college's registrar's office of the school will serve as prrof of continuing eligibility. This needs to be brought to the Health Benefits office at 33 Church Street.

P.E.A. advises its members to have this done now in order to prevent any surprises next time your child needs is ill and needs a prescription. This need only be done once; not every semester or year that the dependent is in college.

PLEASE NOTE: This is not necessary for either medical or dental insurance; only for the prescription plan. Under the State Health Benefits Plan, your dependents are covered until the end of the year in which they turn 23 with or without college attendance, so long as they "live in a regular parent - child relationship" (which means they are dependent upon you for support and don't have their own insurance). The dental insurance covers the child in the same manner until their 24th birthday.

If there are any questions about this procedure, please feel free to contact P.E.A. or Evelyn Mc Leod, Supervisor of Health Benefits for the District at 973-321-0745.

Please don't delay on this matter. Employees, in many cases, do not realize this stipulation at the beginning of the year following the child's nineteenth birthday, and are then taken by surprise when attempting to fill a prescription for their dependent.
