Optical (Vision) CoverageVISION-Administered by Vision Service Plan (VSP)The Vision Service Plan covers only you, the employee. Your eligible dependents are not covered under the vision plan until July 1, 2007. After completion of your vision enrollment application, your vision coverage through Vision Service Plan will become effective 60 days from your date of hire on the first day of the following month. The only exception, 10 month employees who begin work on the first day of school will receive health coverage effective their first day of employment. Simply choose a participating eye doctor from VSP's Web site www.vsp.com which is updated monthly. When you call the doctor for your appointment, tell them that you are covered with VSP. They will do all the necessary paperwork. No vision cards will be issued. However, you can receive a brochure outlining the plan: VSP Customer Service: 1-800-877-7195. Services from an Out-of -Network Provider-If you wish to see an out-of-network provider, VSP will reimburse you up to the amount allowed under your plan's out-of-network provider reimbursement rate. Be aware that your out-of-network provider does not guarantee full payment, and VSP cannot guarantee patient satisfaction when services are received from an out-of-network provider. |
